Process hazards analysis (PHA) is a key element of every successful process safety management program. The purpose of such an analysis is to minimize the number and severity of accidents involving hazardous materials releases. This is achieved by identifying and subsequently eliminating or controlling conditions that could lead to hazardous events.

We commonly employ one or more of the following methods when conducting a PHA:

For "Pre-HA", "HAZOP", and "What if?" studies, Quest provides each Team Leader (facilitator) or Scribe (recording secretary) with a portable computer and suitable software for recording the team findings during the team meetings. We typically use "HAZOP Manager", from Lihou Technical and Software Services, to record these studies. We are familiar with other versions of HAZOP recording software and have used some of them in response to specific requests by clients.

Quest also provides facilitators for Safety Integrity Level studies (SIL) and Layer of Protection Analyses (LOPA), and has done so for numerous clients worldwide. Various SIL software packages have been used by Quest personnel to record such studies, but we typically utilize "HAZOP Manager" modified to use risk graphs identified within codes.

In addition to aiding clients with their Process Hazards Analyses, Quest personnel have conducted numerous training programs to acquaint client personnel with the various PHA methodologies.
